For Case Managers & Referring Professionals

Beacon Support Homes works with case managers, discharge planners, county behavioral health, and treatment providers to place men in structured sober living. Referrals move fast. Same-day intake is possible when a bed is available.

How to Refer

Call (916) 243-5196 to check current bed availability at either home, Elk Grove or North Sacramento. You can also send the referral through the contact form or email info@beaconsupporthomes.com, and the team responds within 24 to 48 hours.

Placement Criteria

  • Men only, at both homes.
  • A minimum of 30 days of sobriety at intake.
  • Enrolled in an active recovery program. AA, Celebrate Recovery, and county recovery programs all qualify.
  • Beacon provides structured housing and peer support. Clinical and recovery services are not provided in-house and are coordinated through third-party providers.

Medication-Assisted Treatment

  • Residents on prescribed MAT are welcome. We facilitate MAT but do not prescribe.
  • Buprenorphine (Suboxone) and naltrexone are supported. Methadone is not supported at this time.
  • Medications can be delivered directly to the house.

Funding

Placements at Beacon are funded through the county's CalAIM Community Supports program, the Medi-Cal initiative that allows managed care plans to cover recovery housing support for eligible members. We work with Medi-Cal and Anthem Blue Cross members, and our admissions team verifies eligibility at no cost.

Certification Status

Beacon Support Homes operates structured, monitored sober living homes and is currently on the waitlist for CCAPP recovery residence certification.

Resident Privacy

We do not publish home addresses, out of respect for resident privacy. Addresses are provided to referring professionals at placement.
